Insurance

I am an in-network provider for Lyra, Premera Blue Cross, Lifewise, Aetna, and First Choice Health/Kaiser PPO. However, you should always call your insurance company and check coverage for your specific plan. For clients with other insurance plans, you can pay me directly and then I can provide you with a “superbill” that you may submit to your insurance company for reimbursement directly from them. The amount reimbursed varies by individual and insurance company.

Here are some questions to ask your insurance company when you call to learn about your coverage:

  • Do I have coverage for outpatient mental health services when I see an out-of-network provider?

  • What is the maximum dollar amount, or maximum number of sessions per year?

  • How much is my deductible? Has it been met this year?

  • Do I have a separate deductible for mental health services? Has it been met this year?

  • What is the coverage amount per therapy session?

  • (If you are in network) What will my co-pay be per therapy session? (You will be responsible for this amount.)

  • How do I submit claim forms for reimbursement from an out-of-network provider?


Fees

Below are the rates I charge for services. These are my standard fees, whether you are paying out of pocket, or planning to submit claims for reimbursement out of network.

  • 50 minute individual therapy session: $200

  • Intake session: $230

  • 15 minute phone consultation: free


Policies

  • For any fees incurred, I accept cash, check, or credit card.

  • You may pay (including co-pays) at the time of the visit, or I can bill you weekly. I ask new clients to provide a card to keep on file, and will charge any remaining balances at the end of each week.

  • If you cancel an appointment with less than 72 hours notice, you will be expected to pay for the full cost, unless there is a truly unavoidable event or emergency.

  • If your insurance company will not pay for a service, you are responsible for the cost — hence the importance of communicating with your insurance company beforehand.

  • My out-of-pocket fees may increase, concordant with my experience and credentials.
